The office will be closed Monday January 2, 2012, our answering service will be available as well as Telekids to take care of any after hours concerns.
Please be aware that we are unable to fax Physical and Sports Physical forms due to privacy regulations. If you are unable to pick these forms up in person please send us a self-addressed stamped envelope and we will be happy to mail them to you.
